No supporting document is required, provided that applicant has been authorized to link up between his/her TSW Registered Account and his/her User Account in Food Trader Portal (FTP).
Supporting Documents Required for Application
- Copy of valid registration certificate of the milk farm issued by a competent authority of the place of food origin;
- Supply certificate issued by the milk farm certifying that it is the direct supplier of the raw milk;
- Certificate issued by a qualified veterinary officer certifying that the milk farm has been inspected regularly and the raw milk comes from healthy livestock not infected with transmissible diseases;
- Schematic layout plan(s) of the entire milk farm and its work areas, and production flow diagram(s) showing in detail all the processes from production to delivery of the raw milk;
- Test report (testing date within the past year) on the quality of water used in the milk farm, including results of coliform bacteria and free residual chlorine levels;
- Test report (testing date within the past year) showing the chemical composition of the raw milk, including results of percentages of milk fat and milk solids other than milk fat;
- Test report (testing date within the past year) showing the microbiological quality of the raw milk, including results of total bacteria count per millilitre and any coliform bacteria in 0.001 of a millilitre;
- Declaration by the milk farm on the shelf-life of the raw milk;
- Details of transportation of the raw milk from the milk farm to a milk factory in Hong Kong, including mode of transport, type of container and its cleaning and disinfection methods, refrigeration and insulation devices, temperature control during the transport process, etc.;
- Methods of monitoring the transport means (such as vehicles) and related equipment, as well as information of the responsible company (including name and contacts);
- Specimen of certificate issued by a competent authority of the place of food origin certifying the raw milk has been produced under satisfactory hygienic conditions and meets specific quality standards; in case legal reference is quoted in the certificate, a copy of the original text of the relevant legislation shall be attached;
- Agreement to supply the raw milk to a licensed milk factory in Hong Kong, together with a copy of the valid licence of the milk factory.
